What is the CMF?

The Clincal Monitoring Form (CMF) streamlines the process of eliciting and recording basic information during routinur follow-up visits from patients suffering from a mood disorder. The goals are simple:

1. Determine the patient's current clinical status and response to treatment.
2. Provide to the patient and their care providers a systematic follow-up to facilitate long term assessment.
3. Track medication use and adverse effects.
4. All in order to assure patients receive high quality care.
